How to Become a Provider

You can become a provider for individuals who have the Level 1 and Individual Options Waiver. The Ohio Department of Developmental Disabilities certifies providers. To obtain information, use the following link, http://dodd.state.oh.us/ and click the provider tab.

After you have been certified, an individual must choose you to be their provider. Once you have been selected, you must provide proof of your certification and professional liability insurance. Individual providers must have CPR, First Aid, Major Unusual Incident (MUI/UI), and Individual Rights Training. An Individual Service Plan (ISP) will be developed naming you as the provider and identifying the specific services to be delivered and the date when service will begin. The Payment Authorization for Waiver Services form (PAWS), completed by the Service and Support Administrator (SSA), authorizes you to deliver and bill for the services you are providing. You are also required to complete detailed records which will document the dates, times, and service provided.
