HOME AND COMMUNITY-BASED WAIVER SERVICES
The Adams County Boards of DD participates in the Home and Community-Based
Waiver Program (HCBS) in accordance with Ohio Administrative Code 5123:2-8,
5123:2-9 and 5123:2-13 and is a provider of the following waiver services:
-Supported Employment
-Adult Day Services
-Vocational Habilitation
-Non-Medical Transportation
Requests for waivers shall be based upon the Board’s available funds for local
match requirement as well as the staff and provider capacity to administer
waiver services.
Under the direction of the Superintendent, the Service and Support
Administrators are responsible for the administration of the Board’s waiver
program.
The Board uses the rules referred to in Paragraph A as the Board procedures for
the HCBS Waiver program and services.
There are two waivers administered by the Board, the Level 1 and Individual
Options Waivers (IO). A description of these waivers can be found on the
Department of Developmental Disabilities website. Individuals requesting waiver
services must be eligible for Medicaid and county board services.
How to Become a Provider
You can become a provider for individuals who have the Level 1 and Individual
Options Waiver. The Ohio Department of Developmental Disabilities certifies
providers. To obtain information, use the following link,
http://dodd.state.oh.us/ and click the
provider tab.
After you have been certified, an individual must choose you to be their
provider. Once you have been selected, you must provide proof of your
certification and professional liability insurance. Individual providers must
have CPR, First Aid, Major Unusual Incident (MUI/UI), and Individual Rights
Training. An Individual Service Plan (ISP) will be developed naming you as the
provider and identifying the specific services to be delivered and the date when
service will begin. The Payment Authorization for Waiver Services form (PAWS),
completed by the Service and Support Administrator (SSA), authorizes you to
deliver and bill for the services you are providing. You are also required to
complete detailed records which will document the dates, times, and service
provided. |
